Social Prescribing 

Health Junction are working with your GP practice to deliver social prescribing support to patients.  Social prescribing offers 1:1 support from a link worker.  A link worker can help you get support with a wide range of social and practical issues, including housing, debt, employment, mental and physical wellbeing and much more.
